Job Description: We are currently seeking a compassionate and dedicated Registered Nurse to join our Community Health team. As a Registered Nurse in Community Health, you will play a crucial role in delivering comprehensive and holistic care to individuals and families within our community.

Responsibilities:

• Conducting comprehensive assessments of individuals and families to identify their healthcare needs and develop personalized care plans.
• Providing skilled nursing care, including medication administration, wound care, and monitoring of vital signs.
• Collaborating with the interdisciplinary healthcare team to coordinate and facilitate continuity of care.
• Offering health education and counseling to individuals and families, empowering them to make informed decisions about their health.
• Assisting in the management and coordination of community health programs and initiatives.
• Conducting home visits and providing care in various community settings, ensuring accessibility and continuity of care.
• Participating in health promotion activities, including immunizations, health screenings, and community education events.
• Maintaining accurate and up-to-date documentation of patient encounters, assessments, interventions, and outcomes.

Requirements:

• Valid Registered Nurse (RN) license and current registration with the appropriate nursing regulatory body.
• Minimum of 2 years of experience as a Registered Nurse, preferably in community health or a related field.
• Strong clinical knowledge and proficiency in delivering nursing care across the lifespan.
• Excellent assessment and critical thinking skills to provide comprehensive and individualized care.
• Effective communication and interpersonal skills to engage with individuals, families, and community stakeholders.
• Ability to work autonomously and as part of a team in a community-based setting.
• Proficiency in using electronic health record systems and other relevant technology.
